PHQ-9 score greater than or equal to 10 has sensitivity of 88% and specificity of 88% for major depression.

Construct validity: association between PHQ-9 scores and functional status, disability days, and symptom-related difficulty.
Criterion validity: assessed against an independent structured mental health professional reinterview, sample of 580 patients
External validity: replicating findings from 3000 primary care patients in second sample of 3000 OBGYN patients
Reliability (internal consistency/test-retest)
Cronbach's alpha = 0.89
Test-retest reliability:  mental health professional conducted interviews by telephone and graded the 9 PRIME-MD questions assessing DSM-IV symptoms, using the same 4 response options as PHQ-9. Correlation r=0.84, mean scores were 5.08 (initial PHQ-9 completed in clinic) vs 5.03 (MHP telephone-administered)

Nine items of PHQ-9 are based directly on nine diagnostic criteria for major depressive disorder in DSM-IV. Can be self-administered or clinician-administered. Limitation: need to establish sensitivity to change
